Palliative Care Leadership Center Training

A health system-based palliative care service providing care to patients at Northwell Health's outpatient clinics, in patient homes, at the Stern Center for Extended Care and Rehabilitation and through targeted services in long term care settings:

• In partnership with the system’s inpatient palliative care services, provides care to patients at NSLIJ’s outpatient clinics, in patient homes, at the Stern Center for Extended Care and Rehabilitation and through targeted services in long term care settings 
• Pioneers in transitioning to shared savings and capitation payment models for palliative care
• Experience training palliative care professionals in leadership skills, including program financing
• Provides patient services in either a co-management or primary care role 
• Certified clinical specialists integrating palliative care practice across the system by training clinical teams in core palliative care skills 
• Uses telemedicine to extend clinical services to patients in the home
