A health system-based palliative care program serving adult patients in home, assisted living and office settings:

  • Oversees interdisciplinary palliative care across the continuum of care, including home and office-based practices
  • Integrated with an inpatient palliative care team, home health and hospice services
  • Innovative program offering Palliative Care in primary care clinics, as well as in specialty settings
  • Demonstrates a shared model of care in offering palliative services as part of a House Calls and Hospital at Home practice
  • Serves patients in urban, suburban and rural environments
  • Partnership with Albuquerque Ambulance and community services organizations to provide continuous coverage and optimal care transitions 
  • Experience in using health system data to identify potential patients; expertise in clinical documentation in the Epic environment
  • Deep experience with new palliative care payment models including a shared capitation arrangement with the system’s health plan, as well as expertise in optimizing fee for service billing
  • Rigorous onboarding and training program for new program staff
