Located at the academic medical center’s cancer center and CHF clinic, the Palliative Care Clinic sees both oncology and cardiology patients in all stages of illness:   

  • Co-located oncology program launched in 2001 and expanded to the University of Virginia CHF clinic in 2014 
  • Recipient of a 2012 CMS Innovation Award for improving care for patients with advanced cancer
  • Work with local American Cancer Society and state hospice and palliative care organizations to provide education and promote awareness of palliative care
  • Integrated with home health, hospice agencies and inpatient palliative care team
  • Focus on measurement of patient outcomes in the medical record to identify patients in need of palliative and supportive care and to guide symptom management
  • Expertise in use of databases derived from electronic medical records to promote ongoing quality improvement and to measure metrics of end-of-life resource utilization
